ANNUAL REPORT: STEP-BY STEP INSTRUCTIONS
YOU MUST FILE AN ANNUAL REPORT
This means you put it in your calendar, you sit down and DO IT every year, just like taxes, or your anniversary, or remembering someone's birthday. YOU HAVE TO DO an annual report.
Step One: Download the blank form.
Download the form, print it, and do it by hand. I know we like to type with our thumbs on our phones, but you should print this form and do it by hand, in your own handwriting. This is for the Judge's benefit. It shows that you aren't just re-using an old form, or using someone else's answers, and shows that you are taking the report seriously.
Step Two: Download the example on how to fill out the form.
Don't copy these answers, use them as ideas for what answers are appropriate. This is to help you figure out how best to answer the questions as it pertains to you and your ward.
Step Three: Make sure to put in the correct dates.
RECOGNIZE: There is a pattern to these dates. FOR EXAMPLE:
Year one's annual report: May 31, 2023, through June 1, 2024
Year two's annual report: June 1, 2024, through May 31, 2025
Year three's annual report: May 31, 2025, through June 1, 2026
Year four's annual report: June 1, 2026, through May 31, 2027
And so on….
Do you see the flip-flopping pattern from one year to the next?
Step Four: Make sure report from physician's office form (last page of the annual report) is completed, make copies (for your records), then file with the clerk.
And that's it! It is much easier than doing your taxes or remembering someone's birthday every year. You can do this and it MUST be done!
