INSTRUCTIONS FOR FILING ANNUAL REPORT

The Annual Report for a Guardian Advocate in Florida is a required document that provides the court with an update on the ward’s condition, well-being, and continued need for the Guardian Advocate. According to the materials provided:

Guardian Advocates must file an Initial Report within 60 days of their appointment and Annual Reports thereafter.

The Annual Report typically includes information on the ward’s physical and mental condition, living situation, and any services they are receiving.

If the Guardian Advocate is handling any property (other than just being a representative payee for government benefits like SSI), a more detailed financial accounting might also be required.
